Important Reminder: Update Your Address for Tax Season
To ensure you receive your 1099 tax form at the correct address, you must update your address in all required systems. An outdated address could affect your payment processing, provider status, and receipt of important tax documents.
Required Address Updates
You must update your address in four separate systems. Please complete these steps in the order listed below:
Step 1: Inform DCBDD
Email your new address to the County Board at provider.relations@dcbdd.org
Step 2: Update Ohio Pays
Ohio Pays is the system that processes your payments. Follow the instructions in this guide to update your address: Ohio Pays Change of Address Instructions
Step 3: Update ODM/PNM

Step 4: Update DODD/PSM

Incident Reporting Requirements
Immediate Reporting
- An Incident Reporting Form (IRF) must be completed for all Major Unusual Incidents (MUIs) and Unusual Incidents (UIs)
- All MUI incidents must be reported and IRFs submitted to MUI@dcbdd.org by 3:00 PM on the following business day
- Use the DCBDD Incident Reporting Form
Monthly Requirements
All providers must complete monthly UI Logs, which are reviewed by DODD and DCBDD during compliance reviews.
Important: Logs must show evidence of monthly review, even when no incidents have occurred. Document your review with a signature and date on the log.

Annual Requirements
Due Date: February 28th (for the preceding calendar year)
By January 31st of each year, all agency providers must:
- Conduct an in-depth review and analysis of MUI trends and patterns from the previous calendar year
- Compile an annual report summarizing findings
Submit the report to MUI@dcbdd.org by February 28th for all programs operated in the county.
Note: This report is required even if you have no MUIs to report.

Background Checks
Who Needs a Background Check?
If you are seeking certification from DODD as an independent provider or as the Chief Executive Officer (CEO) of an agency, you are required to have a criminal background check conducted by the Bureau of Criminal Identification and Investigation (BCI).
What Type of Check Do You Need?
Ohio BCI Check (Required for Everyone):
- All applicants must complete an Ohio criminal background check
FBI Check (Required in these situations):
- You have lived in Ohio for fewer than five years, OR
- You were convicted of a crime in another state, OR
- You were convicted of a federal offense
How to Complete Your Background Check
Step 1: Find a WebCheck Location
- Visit the Ohio Attorney General’s website at https://www.ohioattorneygeneral.gov/
- Search for “WebCheck” or visit the WebCheck Community Listing
- Enter your zip code to find nearby locations
- Important: Choose a location that offers both BCI and FBI services if you need both checks
Step 2: Visit the Location
- Bring a valid photo ID
- Pay the fee directly to the vendor at the location
- Submit your fingerprints electronically
Step 3: Provide the Correct Information
When completing your background check, you must provide the following:
For BCI (Ohio) Background Check:
- Reason Code: 5123.169
- Reason Description: Applicant for a supported living certificate
For FBI Background Check (if required):
- Reason Code: 5126.28
- Reason Description: Employment with DODD

Frontline Supervisor Experience (FLS-X) Training
Meetings available in Columbus on December 10th, at 10 a.m. and 1 p.m.
The FLS Experience brings together Frontline Supervisors to provide valuable feedback and insight to the Ohio Department of Developmental Disabilities (DODD) and system partners, with an aim to improve recruitment, retention, job satisfaction, and career growth.

Waiver Provider Training Series
The last session in our 2025 Waiver Provider Training Series will be held on December 18th at 1 p.m. This in-person training will cover Behavior Supports.
Charting the LifeCourse Side-By-Side Guided Implementation Series
Charting the LifeCourse (CtLC) is a framework created to help people served and families of all abilities and ages develop a vision for a good life, think about what they need to know and do, identify how to find or develop supports, and discover what it takes to live the lives they want to live.
Join the Family Resource Network of Ohio (FRNO) for a side-by-side guided implementation of CtLC for Adult Day Support staff and people with lived experience. This free series offers hands-on practice with CtLC tools in small, supportive groups.
The four-session series will meet select Mondays from 10 a.m. to 11:30 a.m. on January 12th and 26th, and February 9th and 23rd.
Day Program providers can apply for the Side By Side Guided Implementation Series on Microsoft Forms. The deadline to apply is December 23, 2025.

OPRA 2026 Great Provider Summit - Call for Speaker Proposals

Free K-12 Tutoring for Students on IEPs
Learning Aid Ohio is now accepting applications for their Spring 2026 semester (January 10 – May 8).
This program provides free, one-on-one tutoring for K-12 students who:
- Are learning on an IEP through their school district
- Are Ohio residents
- Have a family household income under 400% of the Federal Poverty Level

Health Linkage Program
Adults with disabilities sometimes have worse mental health, physical health, and chronic condition outcomes compared to adults without disabilities.
The Health Linkage Program connects adults with disabilities in Central Ohio to better healthcare and support. Our certified community health worker will meet with you to talk about your health, set goals that matter to you, and help you connect to resources in your community.
The Health Linkage Program can help with:
- Healthcare providers who do not understand disabilities
- Being denied appointments
- Physical inaccessibility of buildings and places
- Inaccessibility of equipment like exam tables
- Inadequate insurance coverage
- Lack of transportation
Who can participate? Adults 18+ with disabilities (including learning disabilities, ADHD, autism, and others) living in Franklin, Delaware, Fairfield, Pickaway, Madison, Licking, or Union counties.

Next Chapter Book Club
Monday, December 15th at 4 p.m.
The Delaware County District Library’s Orange Branch is hosting the Next Chapter Book Club, a welcoming and inclusive reading group for adults and teens with disabilities. The club offers a supportive space to make friends, build literacy skills, and enjoy stories together—no matter a participant’s reading level.
Sessions take place on December 15, January 12, and February 9 at 4 p.m.
World Braille Day Guided Hike
Sunday, January 4 at 10 a.m.
Preservation Parks of Delaware is offering a special sensory experience to celebrate World Braille Day at Hogback Ridge Park! Join them for a free guided hike on the Braille Trail, where you’ll discover the park through touch, sound, and smell.
Led by an experienced naturalist, you’ll feel the unique textures of different plants and trees, listen to the sounds of wildlife, and take in the natural scents of the forest. The Braille Trail is designed to be accessible and engaging for everyone. All ages and abilities are welcome.
