Reporting Major Unusual Incidents

Call the MUI Hotline to report an MUI during business hours at 740-201-3608 or after hours at 740-369-3316 (HelpLine).
For all major unusual incidents, submit a written incident report to the county board contact by 3:00 p.m. on the first working day following the day the you become aware of a potential or determined major unusual incident. Email the incident report to MUI@dcbdd.org or fax to 740-201-3608.
Additionally, all independent and agency providers are required to complete an Annual Review of their MUIs by January 31 and send the completed report to the County Board by February 28. Please use the forms below for the review.

MUI and Individual Rights Training
Major Unusual Incident (MUI) and Individual Rights Training are available for FREE through the MyLearning section of the Ohio Department of Developmental Disabilities (DODD) website.
