Homemaker Personal Care I/O or Level 1 Waiver

Independent young woman seeking 10 hours of homemaker/personal care per week. Preferred schedule is Tuesdays and Thursdays from 10am-3pm. Provider will be providing support in the areas of self-advocacy, budgeting, community inclusion, organization. Female providers only.

Family in Lewis Center is looking for a Homemaker Personal Care provider for their almost 13 year old son to help him with the following: - staying on task and completing personal care routines and chores - community inclusion activities - general supervision and safety. Days and hours needed are Monday thru Friday daily after school (3:15 to 5:30-6)

Young man living in Dublin is in need of an HPC provider. This man has an IO waiver. This man would like a male provider between the ages of 20-40 years old. This man would like a provider to take him into the community to participate in social activities up to 20 hours per week.

Young man's parents are seeking a homemaker personal care staff to assist with daily living tasks. He lives with parents, but requires prompting in order to initiate daily living tasks. They are seeking staff so that they can reduce the number of prompts they are providing and help alleviate the tension that comes with those prompts.

Family in Westerville, is in need of a Waiver Provider for their 7 year old son. He is in need of a Provider who would be able to spend time with him 2-3 afternoons per week from 3:45pm- 5:45pm(or 6:45 pm-times can be flexible). The afternoons/days of the week can be flexible. He is also in need of a Waiver Provider that could be available on Saturdays for 4-5 hours (times can be flexible) to spend time with him. A Waiver Provider is needed who can spend time playing with him, keep him busy with activities, and spend time with him outside. He is able to communicate with words, enjoys active play, being outside, and sensory/movement activities. He has no medical or medication needs, and is becoming independent with self care skills.

Seeking provider for Polaris Area, Monday, Wednesday and/or Thursday after school (some flexibility would be helpful to work around medical appointments), starrting at 5 hours per week. Family is looking for an HPC provider for their 4-year-old child. Provider will help transfer child to a variety of locations per child’s request and ensure safety. Parents will train provider on how to use a variety of mobility equipment and possibly help feed child foods prepared by parents. Provider will help child with communication device and simple signs. Child will need diaper changes and help sitting on the potty to begin potty training. It takes time for child to open up to new people and and then likes to be silly. Child loves to color, play with sensory items, make up silly games and is very good maneuvering wheelchair around the home. Knowledge of Russian language and/or the willingness to learn would be welcomed!

Young man seeking male HPC provider, 20 hours a week, schedule changes week to week. He likes to be out and about in the community, and is also working on further developing his independent living skills.

Lewis Center family is seeking a HPC Provider to work with their 10 year old son. He uses and wheelchair and has a g-tube. Child is generally very happy and enjoys swimming, going out in the community and going to the library. He likes music, watching videos and hanging out on his mat. He has a hoyer lift and adult changing table. Requested hours are M-F 3:30 pm-9:30 and weekend hours 12p-8p.

51-year-old woman, on an IO waiver in the city of Delaware is looking for HPC to come into her home Monday through Sunday for 2-3 hours per day. Provider will assist with medication administration, personal care, community inclusion, assist to any appointments and light housekeeping. Provider should be a female. Services would start 10/01/2023. Medication Administration Cert 1 is required.
