Seeking provider for Polaris Area, Monday, Wednesday and/or Thursday after school (some flexibility would be helpful to work around medical appointments), starrting at 5 hours per week. Family is looking for an HPC provider for their 4-year-old child. Provider will help transfer child to a variety of locations per child’s request and ensure safety. Parents will train provider on how to use a variety of mobility equipment and possibly help feed child foods prepared by parents. Provider will help child with communication device and simple signs. Child will need diaper changes and help sitting on the potty to begin potty training. It takes time for child to open up to new people and and then likes to be silly. Child loves to color, play with sensory items, make up silly games and is very good maneuvering wheelchair around the home. Knowledge of Russian language and/or the willingness to learn would be welcomed!
The Individual/guardian will be provided with the names of the providers who respond to this request. The Individual/guardian will then review the providers who responded and will choose who they interview.
