27-year-old male in Dublin Ohio on a SELF-Waiver is looking for homemaker personal care two days a week for up to eight hours per day (16 hours per week). He is his own guardian and mother is his authorized representative. This young man resides with his parents at this time but is independent for the most part. Looking for a provider who can help the with developing life-long independent skills such as community engagement/participation, assisting with and setting up opportunities for socialization with peers and social communication/interactions, helping with understanding how to meal plan, prep and develop cooking skills, understanding how to manage money and create a budget for earnings vs. expenses. Provide will be giving reminders about personal care and healthy habits, help the individual develop and understand how to organize personal space and shared home areas and develop skills needed to maintain cleanliness for housekeeping to prepare for residing alone. Provider will transport and participate in the young man's preferred activities and give suggestions and prompts to help him decide on what activity he may enjoy that day. He enjoys assembling and painting Warhammer 40K figurines and enjoys welding, playing video games cooking and baking but would be open to expand his interest.
Provider should have experience with all the above and have good communication skills. Provider should have knowledge of activities to help him engage in and have good organization themselves. Provider should be able to provide by example by demonstrating and taking an initiative to create a list of activities or plan out and agenda of things each week. Provider must be willing to engage with him at all times.
Days that services will need to take place will be on Mondays and Fridays with every third Monday the shift starting at 12:00 pm due to the individual having a standing appointment. Provider interested in the position will need to participate in an interview process with the individual, his parents and the SSA.
The Individual/guardian will be provided with the names of the providers who respond to this request. The Individual/guardian will then review the providers who responded and will choose who they interview.
