Family of 17 year old young man with Prader Willi Syndrome is looking for an agency provider to provide 24/7 support in their family home. He attends school Monday-Friday and will not need a provider during school hours. He has a behavior plan that providers would need to be trained on and it would be implemented in the family home. He is very social and friendly. He enjoys engaging with others. He likes to be helpful and likes to please. He's also very good and responsible in caring for dogs. He loves being called the animal whisperer, because most animals love him. He enjoys being out and about and would like his provider to take him to do fun things in the community.
The Individual/guardian will be provided with the names of the providers who respond to this request. The Individual/guardian will then review the providers who responded and will choose who they interview.
