Family of a 13-year-old boy with Autism and Epilepsy is looking for an HPC providers to perform service in the Powell area. Child uses a communication device and behavioral strategies with follow along from behavior support specialist. Child will have access to I/O waiver funding with the behavioral add on rate starting February 2. All HPC services will be locally covered until the I/O waiver starts. Family is requesting provider services during the school year on the schedule listed below:
Monday- No provider
Tuesday -2 Providers 3-9
Wednesday-No provider
Thursday-2 providers 3-9
Friday- No provider
Saturday 2 Providers 10-8
Sunday 2 Providers 9-9
Family is requesting to interview with the staff that will be providing services directly in the home.
The Individual/guardian will be provided with the names of the providers who respond to this request. The Individual/guardian will then review the providers who responded and will choose who they interview.
