Young man and his family are seeking a provider for around 20 hours of HPC services a week. He currently has the self-waiver and is still in high school. They would like staff who are able to get him out into the community to participate in a variety of activities and help him find activities he’s interested in. Also, someone to work with him in the home to develop more independence, skill building, work habits, and daily living skills.
The Individual/guardian will be provided with the names of the providers who respond to this request. The Individual/guardian will then review the providers who responded and will choose who they interview.
