Family in Lewis Center is in need of an HPC provider for their 13 year old son. They would like someone to take him out into the community and to help him participate in different activities that he is interested in, as well as work on communication and social skills while doing so. This individual uses a communication device to communicate with others, and the family would like HPC provider to work with their son on using the communication device more frequently.
During the school months, the family would like for a provider to come to the home 2 times a week. The family is flexible with days, but would prefer one weekday and one weekend day. During the weekday, the family is looking for someone for 3-4 hours a day. On the weekend, the family would like someone to work with their son 6-8 hours.
For the summer months (starting around June 1st, 2023) they would like someone who could come 4-5 days a week, for 6-8 hours each day. Again the days and times are flexible, although the family would prefer at least one of the days on the weekends.
An ideal schedule would be:
School months: every Wednesday 3-4 hours, and Saturday or Sunday 6-8 hours
Summer months: every Monday, Wednesday, Friday and Saturday or Sunday 6-8 hours each day.
The Individual/guardian will be provided with the names of the providers who respond to this request. The Individual/guardian will then review the providers who responded and will choose who they interview.
