The family of a very sweet 10-year-old boy is looking for an HPC provider to work with him over the summer. This young man uses a communication device to express himself, and has great receptive communication skills. The family is looking for someone to work approximately 30-40 hours a week, including possible transportation to appointments. Responsibilities include increasing independence with meal preparation, eating, self-care and other daily living skills, such as responsibilities. Family would also like provider to work with him on socialization skills, to include peer interaction and being out in the community. The family will be on vacation June 18-25 and the individual will be in extended school year June 12-15 and July 17-20, 24-27 from 8:30am-11:30am, so times will change during those weeks.
The Individual/guardian will be provided with the names of the providers who respond to this request. The Individual/guardian will then review the providers who responded and will choose who they interview.
