Family in Sunbury is looking for HPC for their 6 year old son who has the Level One Waiver. Family is looking for staff to work with him on personal care, communication and help with supervision/safety in the home and out in the community. They would like someone to work with him Monday-Friday afterschool (4:00pm) for 3-4 hours a day. More hours available during the summer and winter breaks. He loves playing with peers, gross motor activities, and being outside.
The Individual/guardian will be provided with the names of the providers who respond to this request. The Individual/guardian will then review the providers who responded and will choose who they interview.
