Young woman age 27 living at home with her mother is in search of an Homemaker Personal Care provider. She mainly uses a wheelchair and is also able to be somewhat mobile on the ground. Staff would need to be able to lift at least 80 lbs., know how to use a Hoyer lift and track system. Staff would need to be able to take her out for daily activities, be able to communicate using an electronic communication device (or be able to learn), be Med 1 certified to pass medications, experience with PEG tube nutrition delivery, and have experience safely redirecting occasional physical behaviors. Services would include but are not limited to preparing meals, feeding by mouth, toileting, personal hygiene, some housekeeping tasks, and the ability to provide companionship. Homemaker Personal Care hours needed include Monday-Friday from 4:00PM – 10:00PM and awake over night staffing until 5:00am. 24 hours HPC staffing Saturday and Sunday.
The Individual/guardian will be provided with the names of the providers who respond to this request. The Individual/guardian will then review the providers who responded and will choose who they interview.
