Family is looking for a Homemaker Personal Care Provider for their 6 year old son during the summer months(end of May through mid August). A HPC Provider is needed 24 hours per week (Monday, Wednesday, and Friday from 9am-5pm) during the summer. HPC during the school year is needed 20-24 hours per week, but hours can vary according to Provider's schedule. A HPC Provider is needed to provide interactions, play activities for their son who has Cerebral Palsy, and vision impairment. Provider would need to be able to help with transferring and moving child in his home, and when going places in the community with the child/parent.
He is a playful, friendly little boy who enjoys music, taking walks outside, swimming in the summer, and playing with toy vehicles and blocks. He is able to move with crawling and rolling, and needs constant adult supervision.
Provider that has experience with children who have Jtube feedings preferred, but not required.
The Individual/guardian will be provided with the names of the providers who respond to this request. The Individual/guardian will then review the providers who responded and will choose who they interview.
