Family living south of the City of Delaware is in need of HPC services for their 7 month old son. The family would like the provider to come to the home 2-3 days per week to work on overall development, personal care and therapy goals (OT and PT). The best days and times for the family are Mondays, Thursdays and Fridays from around 9am to 11am.
The Individual/guardian will be provided with the names of the providers who respond to this request. The Individual/guardian will then review the providers who responded and will choose who they interview.
