Family in Lewis Center is in need of HPC/HPC transportation weekly for their 14 year old son. Days of the week needed are Mondays and Tuesdays. There would be 2 options for services. See below.
Option #1 - He would need to be picked up at 5pm from his home in Lewis Center, taken to the YMCA in Powell and then picked back up from the YMCA around 6:30-7pm and taken back home. Provider would not need to stay while he is participating in the class/practice.
Option #2 - He would need to be picked up at 5pm from his home in Lewis Center, taken to the YMCA in Powell and then taken back home after his class that would end around 6:30-7pm. Provider would stay to observe interactions with peers and provide feedback afterwards. Provider would also be there as a support person as needed.
The Individual/guardian will be provided with the names of the providers who respond to this request. The Individual/guardian will then review the providers who responded and will choose who they interview.
