Family in Delaware is looking for an HPC provider for their 9 year old son. They would like for the provider to work with him on OT and PT therapy goals as well as being more independent in the home (personal care and chores). The family would like for the provider to come to the home 2-3 nights per week for around 2 hours per night.
The Individual/guardian will be provided with the names of the providers who respond to this request. The Individual/guardian will then review the providers who responded and will choose who they interview.
