The family of a soon to be 6 year old boy is looking for a provider to work with their son during the summer break. This boy loves to be outside and the family would like the provider to take him outside as much as possible. Outside activities would include playing in the backyard, going on walks and taking him out into the community to places such as the zoo, parks, etc. The family would also like someone to help with his daily living activities as well as help support his occupational therapy goals and his fine motor development. During the summer break, the family would like someone 30 hours a week, Monday through Friday. If an option the family would be interested in working with the provider during the school year. The family will discuss specifics about what school day hours may look like.
The Individual/guardian will be provided with the names of the providers who respond to this request. The Individual/guardian will then review the providers who responded and will choose who they interview.
