The family of a 14 year old girl is looking for a Homemaker Personal Care provider to work with their daughter on personal care skills, communication and daily living activities. Since the Covid-19 pandemic, the 14 year old has shown a preference to stay home but the family would like to see her access the community more often. The family is looking for a provider that can help her access the community, as well as provide support to her when out and about. The family would prefer a female provider and they are open to scheduling. Ideally, they would like someone that could work with their daughter from 3-6pm throughout the week and every other Saturday.
The Individual/guardian will be provided with the names of the providers who respond to this request. The Individual/guardian will then review the providers who responded and will choose who they interview.
