Family in Sunbury is looking for support staff for their almost 6 year old son. The child is on the Level One Waiver. Family is looking for staff to work with him on activities of daily living including help with supervision and safety within the home and in the community. The family would like someone to provide services Monday-Friday afterschool (4:00pm) for 4 hours a day approx. Please reply to this RFP if you are interested in providing this service.
The Individual/guardian will be provided with the names of the providers who respond to this request. The Individual/guardian will then review the providers who responded and will choose who they interview.
