Family in Lewis Center in need of a speech therapist and an occupational therapist (see OT RFPs) for their 13 year old daughter. They are available any day after 3:30, but prefer Monday or Wednesday afternoons. They would ultimately prefer both OT and speech therapy with the same agency and would also prefer back to back therapy sessions if possible.
The Individual/guardian will be provided with the names of the providers who respond to this request. The Individual/guardian will then review the providers who responded and will choose who they interview.
