16 y/o in need of support 3 to 4 days a week. Needs full assistance with daily living skills, (Toileting, bathing, getting dressed, brushing teeth, etc). Enjoys going to the park or taking walks. Is eyes on in the community at all times. Communicates non-verbally and is learning to use a communication device.
The Individual/guardian will be provided with the names of the providers who respond to this request. The Individual/guardian will then review the providers who responded and will choose who they interview.
