Homemaker personal care provider needed to assist 18 year old young lady with activities at home to start. Family has flexibility with hours, but an approximate schedule will be several weekdays between 9:00-1:00, with some weekend hours. Once she is familiar with staff, additional hours can be added, along with short community outings such as going out for dinner, taking a walk, going bowling, etc. She likes to watch movies, play games, and building with her magnets. The long term plan is for her to have a provider until she graduates & after once she transitions to independent living. This will be the first time she has had a provider. Someone with experience supporting people with Autism, willing to be patient to assist her transition to changes gradually, and would like to be part of her life providing long term support would be ideal. Some behavior support training will be part of the provider role, with support from Behavior Support Specialist.
The Individual/guardian will be provided with the names of the providers who respond to this request. The Individual/guardian will then review the providers who responded and will choose who they interview.
Your RFP response may be sent directly to the individual/guardian. Please take this opportunity to make a good impression.
