Woman, in Powell, seeking a provider. She has CP and uses a wheelchair. She is graduating from college in May and is looking for a provider to assist with daily living tasks such as helping her get ready in the morning, doing laundry, helping her with cleaning her place, transportation to get to and from appointments, employment, activities, and groceries and shopping once she returns home from college. She will also need supports at night to fix her food and help her get dressed and be ready for bed.
She is very independent and can make decisions on her own and very driven in finding employment and having activities with friends and family consistently. Currently on a SELF waiver, she owns her own modified/accessible van that her staff can drive for her. She is looking for an agency that will have staff available for her needs and flexible with staffing. She will require staffing 7 days a week.
The Individual/guardian will be provided with the names of the providers who respond to this request. The Individual/guardian will then review the providers who responded and will choose who they interview.
