Intake and Eligibility
Serving families with children birth through seven years:
Referrals originate through the Help Me Grow Central Intake for Delaware County which is housed at the Delaware General Health District, 3 W. Winter Street, Delaware, Ohio. The phone number for Help Me Grow is 740-203-2090.

Supporting families with children ages three through six years:
A child is eligible if they meet one of the following criteria:
  1. Eligible for special needs preschool
  2. A professional diagnosis or a condition known to cause developmental delays
  3. A documented delay based on standardized testing

Supporting youth ages six through high school:
Eligibility for County Board of DD services requires an individual to have a diagnosed developmental disability and three or more areas of substantial functional limitations. The areas that are assessed through state mandated assessment called the (Children’s) Ohio Eligibility Determination Instrument (COEDI/OEDI) are:
Mobility
Receptive & Expressive Language
Capacity for Independent Living
Self – Care
Self – Direction
Learning
Economic Self Sufficiency (at age 16)
